League administration has one golden rule: never coordinate with twelve people when you can coordinate with one. Team registration encodes that rule — the captain fills one form, owns one inbox, and answers for one squad, and the league office deals in teams rather than a spreadsheet of individuals.

Why these fields. Team name comes first because it is the identity everything else hangs on — fixtures, standings, the group chat, the trophy engraving (the placeholder's "keep it printable" has saved more than one league newsletter). Captain name, email, and number form the coordination spine: fixtures and notices flow by email on the captain's schedule, while the phone number exists for exactly one scenario — it is 6:40 p.m., the pitch is flooded, and round three needs to move now. The division question is self-seeding with consequences; offering "social — just for fun" as an honest option is what keeps sandbagging out of competitive and ringers out of social, because teams sort themselves accurately when the fun option carries no shame. The roster long-text with one player per line is deliberately low-tech and export-friendly — it reads cleanly in the responses view and pastes straight into a league sheet. The squad-size number bounds it with your league's minimum and maximum, catching the seven-player "eleven-a-side" entry at submission rather than at forfeit. Kit color is the question leagues forget until two teams in navy meet in round one; collected here, the fixture planner schedules clashes apart or warns captains to bring bibs.

What we left out. Per-player emails, birthdates, and medical notes — those belong on individual player forms where each person answers for themselves; a captain guessing teammates' details produces confidently wrong data. League fees also stay off: invoice per team through your usual channel, one line per entry in the export.

Who uses this. Sunday-league soccer and netball associations, corporate sports days, quiz-league and esports organizers, dodgeball and kickball socials, and university intramural offices that live in team units rather than player lists.

Make it yours. Rename divisions to your real ladder and set the roster bounds to your format's legal squad sizes. Set a close date at your entry deadline, and add a webhook so each new team posts straight into the league committee channel — watching the divisions fill is half the preseason fun.
