For hourly and high-volume roles, the application form is a funnel problem before it is a screening problem. The best cashier you'll hire this year is applying from a phone during a bus ride, and every extra required field is a reason to bail. This template is engineered around that reality: seven inputs, one open question, and a resume field that is deliberately optional.

Why these fields. Name, email, and phone are the callback envelope — and the phone number carries a note that you'll call or text, which sets expectations and raises answer rates when you do. The role dropdown keeps one form serving every opening at the location, so your "now hiring" poster needs only one link. Availability is the true screening question for shift work: a candidate who can only do weekday daytimes is a different hire than one open to weekends, and a multi-select answers it in two taps. The single pitch question replaces the cover letter; its placeholder explicitly invites plain talk, because you're screening for reliability and attitude, not essay craft. The resume stays optional on purpose — plenty of excellent hourly candidates don't have a current resume, and requiring one filters for document-formatting skill rather than job skill.

What we left out. Work history grids (you'll cover it in a ten-minute interview), references (ask the finalists), start-date pickers (shift scheduling moves faster than form fields), and anything that smells like a corporate applicant tracking system. The form renders one question at a time in Focus mode, which feels closer to messaging than paperwork on a phone screen.

Who uses this. Restaurants and cafes taping a QR code to the door, retail stores hiring for seasonal peaks, warehouses and delivery hubs with rolling openings, and franchise managers who need one link that works across locations.

Make it yours. Swap the role options for your actual openings and delete the ones you're not hiring for — an empty-feeling dropdown reads as a dead posting. Print the share link as a QR code for the counter or window; applicants can finish before their coffee is ready. Turn on email notifications so new applications interrupt you the day they arrive — speed-to-contact is the whole game in hourly hiring, and the first employer to call usually wins. If the same person tends to apply twice, enable duplicate prevention in Settings to keep the pile clean.

A note on tone. Every string on this form — the intro, the placeholder, the ending — is written like a human note rather than HR copy. Keep that register when you edit. The kind of applicant who reads "we check applications daily" and believes it is exactly the kind who shows up on time for the interview.

No — the upload is optional by design. If you want it required, flip the toggle on the block, but expect fewer completed applications for hourly roles.

Does this work well on phones?

Yes. Focus mode shows one question at a time with big tap targets, which is the fastest pattern for mobile applicants coming from a QR code or social post.

How do I know the moment someone applies?

Enable "Email me on every response" in Settings. In hourly hiring the first call usually wins, so same-day contact is worth the notification noise.

Can I stop duplicate applications from the same person?

Turn on duplicate prevention in Settings — it can block repeat submissions per device or per IP, which keeps enthusiastic double-appliers from cluttering the list.
