The difference between a photography release and a media consent form is scope: modern content production captures a person's image, motion, voice, and words in a single afternoon, and publishes each through different channels with different stakes. A blanket "we can use everything everywhere forever" signature is easy to collect and corrosive to trust — and increasingly out of step with how privacy-conscious participants read forms. This template replaces the blanket with a menu.

Why these fields. Naming the project is the load-bearing decision here: consent tied to "the spring fundraiser video" is comprehensible and time-boxed, while consent tied to nothing sprawls into everything and is precisely what people hesitate to sign. The channel multi-select is the heart of the form — website, social, print, paid advertising, and internal use each carry different exposure, and paid advertising in particular deserves its own explicit tick because appearing in an ad is categorically different from appearing in a newsletter. Requiring at least one selection keeps the record meaningful. The under-18 gate conditionally reveals guardian name and relationship fields, so the same form serves a community theater's adult cast and its youth ensemble without maintaining two documents — and the signature line is worded to bind whoever signs, subject or guardian.

The guardian gate, in practice. The under-18 question deserves care beyond the form itself. Only a parent or legal guardian can grant this consent — the relationship selector exists to stop a coach, an older sibling, or a well-meaning aunt from signing in the rush of a filming day. Before publishing anything with children in frame, filter responses for a yes on the minor question and confirm each one carries a guardian name; a record where those two fields disagree is a record to fix, not to file. Compensation clauses (paid releases are contracts and belong with your legal process), perpetual-and-irrevocable language (the scope statement promises withdrawal for future use, which is both kinder and more collectible), and biometric or model-registry fields that overreach for organizational use.
